How can I create a Subscription form?

You can create a Subscription form by following these steps : 

Login > Marketing Tools > Overview > Subscription > Subscription Forms > Create Form               

Give the Form Name, any Tags you want to associate to people who provide their details via this form and the Lists you want to add these contacts to.
